`SAML_PROXY_NAME_ID_MAPPING` does not work with forward auth mode

It seems that SAML_PROXY_NAME_ID_MAPPING only works when using saml-auth-proxy as a proxy but not when using it in forward auth mode.

Can you clarify what you expected to happen? Should the name ID headers be added to the auth response?
