Document our release process

While most our release process is automated (awesome) there are still manual steps involved. We should:

1. Documentation how the automation works
2. Document the manual steps (potentially writing an upgrade guide, updating release notes with upgrade guide, creating blog entry even if its just a link to the release notes)

In particular, let's be clear about what goes in the "github release notes", changelog, and blog entry.

Though I have an outsider's perspective, I would recommend that the changelog entry and GitHub release notes be copies of one another.
