Add a `CONTRIBUTING.md`

We should add contributing instructions in a CONTRIBUTING.md file (that filename specifically means GitHub automatically links to it from various places).

I'm not sure if CONTRIBUTING.md might be the right place for it, but a community slack, discord, or forum would also be great to help foster discussion and contributions.
