Cosmetic interface improvements

Here are some of the improvements to Grist's GUI that would be welcome IMO:

• when adding a document, ask for the document name upfront (instead, now, one has to go back to the home page and change the document name, which is definitely not cool)
• when addng a table, ask for the table name upfront by selecting its name in the list, a bit like what you do when adding a new column

On the first bullet - the document name can be edited in place:

I agree we could certainly do more to highlight this, or nudge user to set on document creation.

Good point about tables!
