I think that that #47 nearly fixes this with some small chances to inline.ts. I'll commit those to my fork now.

I think I can do with with #47. I have some code but need to test a bit more.
I believe #49 would help here since you could [use a plugin](https://www.npmjs.com/package/rehype-toc) to add a TOC in post processing.
