Move `model` -> `ember-data`

We've renamed Models to Ember Data, but the URLs haven't been updated just yet. We should update them to reflect the new naming.

It was initially avoided because of the redirects, but with the Octane changes that doesn't really matter. We should get this done for 3.15.
