Echo: anti-click when adding/removing taps

Adding or removing taps causes clicks. It should be possible to use a Smoother with a delayed action to eliminate these clicks. Instead of immediately inserting/removing the new tap, start ducking, then perform the action, then ramp back up. The ducking has to be performed to any signal that could be affected, which pretty much means the entire chain.

Clicking is likely to be more of an issue in serial mode than parallel mode, but both must be tested.

I don't know if this is important enough for initial release. I think I'm willing to defer this one until a later release. My belief is that inserting and removing taps are not likely to be desirable performance actions anyway; they are patching operation. Therefore, the development time is lower priority than getting the initial release ready.
