Add option to use setext-style of headers

When I use Format -> Heading or I press ctrl + number to create a heading, the atx style (with #) is always used, even for levels 1 and 2.

It would be nice, if there was option in config to use the setext style (underlined with = or -) at header level 1 and 2.
