Improve documentation on how Pull Requests should be submitted

If there are certain things required from a pull request then a quick note would be great.

I've submitted a pull request and just followed: everything builds still and updated test so that it confirmed that the fix worked.

Cheers. No need to apologise. I appreciate you putting in the time on an OS project and I know it can be be quite demanding at times (especially when you have to balance a day job and personal life alongside it!).
