Bug Report: Contact Us page not working

Reproduction steps

When contact us page is accessed, the validation for it is not working and the data is also not submited.

Expected behavior

When the user clicks on submit button, validation is not working and the data is also not submitted.

Actual Behavior

When the user submits his/her data, the form should be validated and the data should be inserted in the database.

Can you please let me know how can I access the database as I need to create a relation for data to be submitted. The rest of the code is done but it shows me an error that the realtion does not exist.
