Getting the same error. Maybe I can give you a little more info.

Only happens for 1 domain in the account, the rest work. Odd thing is that the app was able to generate a cert for a subdomain on that domain. Line 718 is in the authz function so I thought it may be htaccess related so I renamed the htaccess and still did not work.
