Annotation popovers don't show context parameter for higher level annotations

Annotation popovers are designed to show a context string at the top of each popover like "Attribute length_mm contains Measurements of Type Length". @laurenwalker discovered what looks like a bug in the code that renders this context string for a <dataset>-level annotation. Between "Dataset" and "De-Identified...", the dataset's <title> should be displayed but isn't:
