Multi-GPU process information not available for all GPUs

The process information being displayed is only for the first (primary) GPU. Add details as to which GPU the process is on and collect the process information for all GPUs

Added support for multi-GPU details. This information is not serialized into the GPD file. Due to the limited space I reduced the header labels. ID = GPU Index GP = GPU SM Utilization MC = GPU Memory Controller Utilization EN = GPU Encoder Utilization DE = GPU Decoder Utilization image
