multi-controlled-toffoli gates and their family, what an headache. there are several gates that when added 2 or more controls they don't behave as expected. most of the time they just ignore some of the control wires.

Each type of gate has a limit for number of controls. For X gates the limit is 2. Please see: https://github.com/JavaFXpert/quantum-music-playground#understanding-control-gate-modifiers
