Co-Op: Spawned enemies (lost souls) don't count towards kill tally if they die upon spawning
In co-op, if a lost soul dies immediately upon spawning from a pain elemental, it does not count towards the kill count. This happens when the lost soul runs into the player or a wall when spawning.

I think what I'll do for this situation is assign the kill to player being targeted by the lost soul, which is inherited from the parent pain elemental. Most of the time this will be the player indirectly responsible for the kill. If for some reason that can't be determined (e.g infighting) then I'll assign the kill randomly to one player or the other (50/50 chance of getting the kill).
The changes mentioned have now been implemented as of 1.0.0.
